I understand that dogs are social animals and that interaction with other dogs and people involves inherent risks. These risks include, but are not limited to:
• Bites, scratches, or other injuries
• Illness or communicable disease
• Stress or anxiety
• Escape attempts
• Injuries resulting from rough play
• Allergic reactions
• Death in rare circumstances
I voluntarily accept these risks and choose to have my dog participate in daycare and/or boarding at Play All Day Doggie Daycare.
Health Requirements
I certify that my dog:
• Is current on all required vaccinations.
• Is free of contagious illnesses to the best of my knowledge.
• Is free from fleas, ticks, and other parasites.
• Has not shown aggressive behavior that would endanger people or other dogs unless disclosed below.
Behavior Concerns
Please disclose any known behavioral concerns, aggression, anxiety, reactivity, escape tendencies, or other issues that may affect the safety of your dog, other dogs, or staff.
Group Play
I understand that my dog may participate in supervised group play with other dogs.
Although staff closely supervises all playgroups, I understand that injuries can occur despite reasonable care and supervision.
I understand that Play All Day Doggie Daycare may separate my dog from group play if staff determines that separation is necessary for safety, health, or behavioral reasons.
Emergency Veterinary Care
If my dog becomes sick or injured and I cannot be reached promptly, I authorize Play All Day Doggie Daycare to seek veterinary treatment on my dog's behalf.
I agree to be responsible for all veterinary expenses incurred on behalf of my dog.
Medication
If medication is provided, I understand that staff will administer it according to my written instructions.
Play All Day Doggie Daycare is not responsible for complications resulting from inaccurate instructions, incorrect medication information, or undisclosed medical conditions.
Personal Property
I understand that blankets, toys, leashes, collars, food containers, bedding, and other personal belongings may become lost, damaged, chewed, or soiled.
Play All Day Doggie Daycare is not responsible for the loss of or damage to personal property.
Owner Responsibility
I certify that all information I have provided regarding my dog is true, accurate, and complete.
I agree to notify Play All Day Doggie Daycare of any illness, injury, behavioral change, exposure to contagious disease, or other condition that could affect my dog's participation in daycare or boarding.
Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge Play All Day Doggie Daycare, its owners, employees, contractors, agents, and volunteers from claims, demands, damages, liabilities, costs, and expenses arising from my dog's participation in daycare, boarding, transportation, or related activities, except where caused by gross negligence or willful misconduct.
This release includes, but is not limited to, claims arising from:
• Dog-to-dog interactions
• Dog-to-person interactions
• Minor injuries
• Illness or communicable disease
• Property damage
• Escape despite reasonable precautions
• Emergency veterinary treatment authorized under this agreement
Indemnification
I agree to defend, indemnify, and hold harmless Play All Day Doggie Daycare, its owners, employees, contractors, agents, and volunteers from claims, damages, attorney fees, costs, or expenses arising from my dog's actions while in its care, including injuries or damage caused to other animals, people, or property.
Cancellation, Removal & Dismissal
I understand that Play All Day Doggie Daycare reserves the right to refuse service, separate, remove, or permanently dismiss any dog whose behavior presents a safety concern or whose health may place other pets, customers, or staff at risk.
Entire Agreement & Acknowledgment
By signing this agreement:
• I acknowledge that I have read this agreement in its entirety.
• I understand its contents.
• I understand that participation in dog daycare and boarding involves inherent risks.
• I understand that by signing this agreement I may be giving up certain legal rights.
• I confirm that I am signing this agreement voluntarily.
